Why testing alone misses hormone changes

During perimenopause, oestrogen does not fall in a straight line. Instead it swings, sometimes higher than a normal cycle and sometimes sharply lower in the same week. A single blood sample therefore catches one moment in that pattern. Because of that, we diagnose perimenopause clinically, from your symptoms and age. Meanwhile testing rules out an underactive thyroid or low iron, which mimic the same picture.

What we measure

First we check thyroid function, morning cortisol, and sex hormones drawn on the correct cycle day. Then we add ferritin and vitamin D, since both fall low easily and correct easily. Managing hormone changes

Basics come first. Eat enough protein, train with resistance to protect bone and muscle, control the caffeine and alcohol that trigger flushes, and keep a fixed wake time. Hormone therapy suits some people, although suitability depends on your age, your medical history, and how long since your last period. That said, it is a risk and benefit conversation with a doctor rather than a decision you take from an article.
