Quick Answer

Perimenopause can begin ten years before periods stop. Most symptoms labelled as stress in your forties are hormonal.

Changes in menopause hormones can begin ten years before periods stop completely. That phase is called perimenopause, and most symptoms people label as work stress or ordinary ageing in their forties actually come from hormones swinging unpredictably.

Menopause hormones and the symptoms people miss

  • Sleep that breaks at three in the morning, although falling asleep is easy
  • Anxiety arriving for no reason, usually in the second half of the cycle
  • New joint aches and morning stiffness
  • Brain fog and trouble finding words mid-sentence
  • Weight settling around the middle although diet has not changed
  • Cycles shortening first, then turning irregular

Why blood tests sometimes look normal

During perimenopause, oestrogen does not fall in a straight line. Instead it swings sharply, sometimes higher than an ordinary cycle and sometimes dropping fast. A single blood sample therefore catches one moment in a turbulent pattern. So results can look normal on the day even while symptoms are obvious. Diagnosis is clinical, based on symptoms and age, and testing rules out other causes such as thyroid problems or anaemia.

What to measure

A useful panel covers FSH and estradiol drawn on the correct cycle day, TSH with FT4, ferritin, vitamin D, and a basic metabolic profile. Book a hormone profile test for the full picture. Thyroid matters because an underactive thyroid overlaps almost completely with perimenopausal symptoms, although the treatment differs entirely.

Managing menopause hormones

Start with the basics: enough protein, resistance training to protect bone and muscle, control of the caffeine and alcohol that trigger flushes, and consistent sleep timing. Hormone therapy is an option for some women, and suitability depends on medical history, age, and how long since your last period. That said, it is a risk and benefit discussion with a doctor rather than something to decide from an article.

Next steps

Record your symptoms and cycle dates for two months before your consultation. That record is usually more useful than any single blood test. Read the hormones and midlife pathway. Women health information is published by the Ministry of Health Malaysia. Book a consultation.
