Maintaining Weight Loss
A separate phase after weight loss, with periodic measurement and a written plan for holding the result.
Maintenance is a separate phase with different requirements from the loss phase. Most regain happens within twelve to twenty four months of losing weight, and it happens most often when nobody planned for this phase at all.

What this addressesWeight & Metabolism
Who this is for
- You have just reached your target weight and are not sure what comes next
- You have lost weight before and it returned
- Hunger signalling increased after loss and has stayed elevated
- You want a weight range you can manage, not a single number to chase every morning
How it runs
- 01A weight range, not a single numberDaily weight moves several kilograms for reasons that have nothing to do with body fat. A range gives you a clear action point without making every morning feel like a test.
- 02How often you weigh and measure is setPeople who measure regularly catch movement earlier, while it is still easy to correct.
- 03A written recovery plan, made in advanceWhat you do when weight rises by three kilograms, written down while you are calm rather than decided in the moment you panic. This is the part most often left out.
- 04Resistance training continuesMuscle held is the largest difference between maintenance that works and maintenance that does not.

Testing usually done first
Not every test here is ordered for everyone. The consultation decides which are needed.

Why this phase is handled separately
| Loss phase | Maintenance phase | |
|---|---|---|
| Target | Clear and measurable | No single end point |
| Feedback | Frequent | Infrequent |
| Momentum | Present | Absent |
| Energy the body needs | Higher | Lower, because a lighter body needs less energy |
| Hunger signalling | As usual | Tends to increase and can stay elevated for a long time |
How reviews are scheduled
Reviews happen at longer intervals than the loss phase, usually every three months in the first year. Body composition is remeasured, not weight alone, because stable weight with falling muscle is not a good outcome even though the number looks the same.
| When | What happens |
|---|---|
| The first twelve months | The phase that matters most, and the window planned actively |
| Every three months in the first year | Review. Body composition is remeasured, not weight alone. |
| After the first year | An annual review is usually enough for most people |
Seven strategies for maintenance and rebound prevention
- A weight range rather than a single number, with a defined upper trigger that calls for action
- Protein and strength training held at loss-phase levels, not reduced because the target was reached
- Calories raised in steps and on purpose, not all at once when the plan ends
- Weekly weighing plus a monthly body composition check, so any gain is identified as fat or as muscle
- A re-entry rule written in advance: what happens if the upper trigger is crossed, and who to contact
- Metabolic markers kept under review, not weight alone, since blood sugar and lipids can move before weight does
- Where medication was used in the loss phase, a planned reduction with a follow-up schedule rather than stopping outright
Limits and who this does not suit
Regaining a little is ordinary and is not failure. What is managed is direction and rate, not daily fluctuation.
- Regaining a little is ordinary and is not failure
- What is managed is direction and rate, not daily fluctuation
- If weight returns quickly and substantially despite the plan being followed, that warrants investigating again rather than trying harder
- You do not have to weigh yourself every day. For some people weekly beats daily
- If the loss phase involved medication, the plan for afterwards is written before the medication stops, not after.
Common questions
How long does this phase run?
The first twelve months matter most and that is the window planned actively. After that, an annual review is usually enough for most people.
Do I have to weigh myself every day?
Not necessarily. The frequency is set from what is useful for you without becoming a burden. For some people weekly beats daily.
What if I have already regained a lot?
Then you can start again, and plenty of people are in that position. The starting point only changes the plan, not whether it is worth doing.
If the loss phase involved medication, when is the maintenance plan written?
Before the medication stops, not after. Hunger signals tend to rise once it stops, so the plan needs to be in place already. See doctor-led GLP-1 and weight management.
How long does the maintenance phase run?
It has no fixed end date, because energy needs change after weight comes down. Why weight returns after stopping treatment explains the mechanism. The health-risk background for body weight sits in the WHO fact sheet.
What is the next step?
Maintaining weight loss is easier when the loss phase is closed properly. Book a consultation or read weight treatment stages. For the full picture of the doctor-led programme and where GLP-1 therapy fits, see doctor-led GLP-1 and weight management.
Every iLIV guide is written and reviewed under our editorial standards, and each one carries the name of the reviewing doctor together with their MMC registration number.
Treatments on the same pathway
Not all of these suit everyone. The doctor assesses first.
