Food Allergy and Tolerance Test at iLIV Clinic Damansara Perdana
Quick Answer

Food allergy and food intolerance are two different things, and we test them in different ways.

A food allergy test and an intolerance panel are not the same thing, although people mix them up constantly. Allergy involves an IgE immune reaction that can appear within minutes and sometimes threatens life. Intolerance is slower, vaguer, and does not run through the same immune pathway.

What a food allergy test covers

  • Specific IgE for foods common in Malaysiaprawn, crab, peanut, egg, milk, wheat, fish
  • Total IgE and eosinophil count
  • IgG panel for pattern spotting, not diagnosis
  • Hydrogen breath testing for lactose tolerance where indicated
  • Basic gut markers when symptoms point more to digestion

What it cannot do

Limits of a food allergy test

Testing cannot explain every digestive symptom. Persistent bloating sometimes comes from H. pylori, bacterial overgrowth, or gut motility rather than a particular food. If your symptoms point that way, a stool test suits better. Reference information is published by the Ministry of Health Malaysia. Book a consultation.

Allergy versus intolerance

A true food allergy triggers IgE antibodies. The symptoms are fast and obvious: swollen lips, rash, breathlessness, vomiting. Intolerance instead gives bloating, fatigue after eating, or a headache hours later. Its cause is often not immune at all, but a missing enzyme such as lactase, or sensitivity to compounds such as FODMAPs.

An honest note on IgG panels

Food IgG panels are popular although the evidence is weak. IgG antibodies to a food can simply mean you have eaten it, not that you react to it. So we use IgG results as one pointer for structuring an elimination trial rather than as a permanent banned list. Anyone selling you forty forbidden foods on IgG alone is overpromising.

Structured elimination

For most intolerance cases, structured elimination is still the most reliable method. We remove the suspected foods for three to four weeks, then reintroduce them one at a time while recording symptoms. We build that schedule with you, so your nutrition stays adequate throughout. Read the gut health pathway and the guide to gut problems.

Frequently Asked

Can I trust IgG testing?

The evidence is weak. IgG shows you have eaten a food, not that you react to it. We use it as a pointer, never as a banned list.

Do I need to fast?

No. If you take antihistamines, stop three to five days beforehand after discussing it with the doctor.

How long does elimination take?

Three to four weeks off, then reintroduce one food at a time.
