Weight Treatment Stages
A structured approach to weight, built after consultation and objective measurement rather than from a package chosen in advance.
Weight management at iLIV starts with a consultation and measurements, not with a package. The doctor reviews your history, what you have already tried, and your test results before any plan is written. What we measure is not weight alone but body composition, so that muscle you lose is never mistaken for progress.

Who this is for
Weight management at iLIV is built after measurements are taken, not before. This page explains what happens from the first consultation through to review. Most people who come to iLIV about weight are not people who have never tried. They have tried, often several times, and something stopped working. Three patterns come up most.
- Weight climbing slowly every year although eating habits have barely changed
- Weight that drops at the start, then flattens completely however much effort is added
- Weight that came off successfully, then returned within one to two years
- You have tried several times and something stopped working
How it runs
The consultation comes first. Around 45 minutes, and no testing is ordered at that stage. What the doctor collects is your weight history across your life, not only where it sits now. When it started rising, what was happening in your life at that time, what has worked before and how long it lasted.
- 01Consultation first, around 45 minutesNo testing is ordered at that stage. What the doctor collects is your weight history across your life, not only where it sits now, including current medication, since several common drug classes taken for other reasons do affect weight.
- 02Only tests that change a decisionTests that change nothing are not ordered. For most people that means body composition and a metabolic panel. A hormone or thyroid panel is added where the history gives a reason to look there.
- 03The plan is written from the resultsNot from a standard list. Every item in the plan has a stated reason, and that reason points to something in your results rather than to a package.
- 04Starting treatmentThe starting dose is deliberately low, and the doctor explains what the first weeks should feel like and what needs reporting straight away.
- 05MonitoringWeight, body composition and symptoms are reviewed on a set schedule rather than when something feels wrong. Blood panels are repeated where there is a clinical reason.
- 06Dose optimisationThe dose goes up only where response and tolerance say it should. If the current dose is still moving things, it stays.
- 07Muscle preservationProtein, strength training and body composition measurement run alongside treatment. Weight lost without holding muscle comes back more easily.
- 08Plateau managementWhen movement stops, the plateau is investigated first. Raising the dose or cutting calories is not the first step.
- 09MaintenanceThe phase after the target is reached is handled separately, with a written plan and periodic review.

Testing usually done first
Not every test here is ordered for everyone. The consultation decides which are needed.

Why body composition matters more than weight
A scale gives one number, and that number combines several things moving in different directions. Muscle, fat, water and gut contents all sit inside the same figure.
| The scale | Body composition test | |
|---|---|---|
| What it measures | One number combining everything | Fat, muscle, water and gut contents separated |
| Lost 5kg with muscle held | Same number | Visible |
| Lost 5kg with muscle lost too | Same number | Visible |
| Effect on what comes next | Cannot be read | Lost muscle lowers resting metabolic rate, so weight is easier to regain |
What usually changes, and when
Energy and hunger patterns tend to move first, often within two to four weeks. That change is usually felt before it is visible.
| When | What usually moves |
|---|---|
| Week 2 to 4 | Energy and hunger patterns. Usually felt before it is visible. |
| Week 4 to 6 | First review. It is there to read direction, not to hand out a grade. |
| Week 8 to 12 | Body composition. Metabolic markers in blood are repeated on the same schedule. |
Limits and who this does not suit
Not everyone is suitable for every component. Pregnancy, breastfeeding, several active medical conditions, and a history of eating disorders all change what can be offered, and some rule certain approaches out entirely.
- Pregnancy and breastfeeding
- Several active medical conditions
- A history of eating disorders
- We do not set weight targets from the outside. Targets are agreed together, from what can be sustained in your actual life rather than from a chart
- If your weight is falling without you trying to lose it, that is a different matter and it needs investigating sooner, not managing as a programme
- GLP-1 therapy may be part of the plan if the doctor judges it clinically appropriate. It is not the basis of the programme, and eligibility is decided after consultation and testing.
Common questions
How long before I see something?
Energy and hunger within two to four weeks. Body composition at week eight to twelve. If nothing has moved by week eight, that is information, and the plan is adjusted on it.
Do I need tests before the consultation?
No. Consultation first. Testing is ordered once the doctor knows what needs answering. If you have older results from another clinic, bring them, because it may mean some tests do not need repeating.
What happens once I reach my target?
The maintenance phase is part of the plan, not an add-on. It is handled separately because what it requires is different from what the loss phase requires.
Does the programme include GLP-1 medication?
It may, if the doctor judges it clinically appropriate after consultation and testing. It is not the basis of the programme and it is not for everyone. The full explanation is on doctor-led GLP-1 and weight management.
Is there an independent reference on weight and health risk?
The WHO fact sheet explains how body weight relates to metabolic risk without reference to any clinic. For the baseline we measure before anything starts, see the metabolic assessment.
What is the next step?
Weight management starts with one consultation. See the tests that may be involved or read the weight and metabolic health pathway. For the full picture of the doctor-led programme and where GLP-1 therapy fits, see doctor-led GLP-1 and weight management.
Every iLIV guide is written and reviewed under our editorial standards, and each one carries the name of the reviewing doctor together with their MMC registration number.
Treatments on the same pathway
Not all of these suit everyone. The doctor assesses first.
