What happens to cortisol
Cortisol should peak within an hour of waking, then fall through the day to its lowest around midnight. When that pattern inverts or flattens, you feel tired in the morning and alert at night. Measuring morning cortisol gives an early pointer, and we read it alongside TSH and ferritin, because iron deficiency and an underactive thyroid produce almost identical symptoms.
Sleep apnoea gets missed constantly
Sleep apnoea is not only a problem for people carrying extra weight. Jaw structure and neck size contribute too. So if there is loud snoring, a choking sensation during sleep, or heavy daytime sleepiness, screening comes before any hormone investigation. Treating hormones while apnoea is the real cause will not work.
Basics that genuinely shift sleep and stress
Keep a fixed wake time every day, weekends included, because that does more than a fixed bedtime. Get morning daylight within thirty minutes of waking, since it helps set your body clock. Take your last caffeine before midday, as its half-life runs five to six hours. Alcohol shortens REM sleep, although it does make falling asleep easier.
When to test
If you have corrected the basics for four weeks and nothing changed, that is the point to measure.
