Who Is Eligible for GLP-1 Therapy

Eligibility is assessed from BMI using Asian cut-offs, waist circumference, and weight-related conditions such as prediabetes or high blood pressure. It is not used in pregnancy or breastfeeding, and not used where there is a history of pancreatitis or medullary thyroid carcinoma.
GLP-1 therapy is not for everyone who wants to lose weight. It is a prescription medicine, so eligibility is decided by a doctor after an assessment, not by the number on the scale.
What is assessed before therapy is considered
The assessment starts with BMI and waist circumference. For Asian populations the BMI cut-offs used are lower than Western ones, because metabolic risk appears at a lower body weight. The doctor also looks for weight-related conditions already present: prediabetes, type 2 diabetes, high blood pressure, abnormal lipids, fatty liver, sleep apnoea or PCOS. The thresholds themselves are set in Malaysian clinical guidelines, and the assessing doctor will explain where your numbers sit.
Who is not eligible
Some situations make this therapy unsuitable or rule it out entirely.
- Pregnant, trying to conceive, or breastfeeding
- Personal or family history of medullary thyroid carcinoma, or MEN2 syndrome
- History of pancreatitis
- Severe delayed stomach emptying
- Known allergy to the active ingredient
- Type 1 diabetes, since this is not the indication
A history of disordered eating also needs assessment first, because a medicine that suppresses appetite can mask a problem that should be treated differently.
If you take other medicines
The two combinations that need the most attention are insulin and sulfonylureas. Both can push blood sugar too low when combined, so existing diabetes doses are usually adjusted at the same time. Other oral medicines can be affected because the stomach empties more slowly, and that is reviewed one by one.
When the answer is not GLP-1
Some people arrive with weight that will not move, but the real cause sits somewhere else. An underactive thyroid, high cortisol, chronic short sleep, or a medicine that drives weight gain can all look identical from the outside. That is why the assessment starts with bloods rather than a prescription. See the metabolic assessment and hormone profile for what gets checked. Read GLP-1 safety and GLP-1 side effects as well. Book a consultation for an assessment. Medicine registration status can be checked through NPRA.
Eligible is not the same as recommended
Someone can meet the criteria and still be better off starting without medicine, for instance when sleep, protein and training have never been addressed properly. GLP-1 therapy manages appetite. It does not manage muscle, sleep or stress. Leave those out and part of the weight lost comes from muscle, and it returns once the therapy stops.

Every iLIV guide is written and reviewed under our editorial standards, and each one carries the name of the reviewing doctor together with their MMC registration number.
Frequently Asked
Do I need a specific BMI to qualify?
BMI is one of several criteria, and the cut-offs used for Asian populations are lower than Western ones. Waist circumference and weight-related conditions such as prediabetes or high blood pressure are assessed alongside it. The doctor will explain where your numbers sit.
Can I take it while pregnant or breastfeeding?
No. GLP-1 therapy is not used during pregnancy, while trying to conceive, or while breastfeeding.
What if I already take diabetes medicine?
It can still be considered, but doses need reviewing. Insulin and sulfonylureas carry a low blood sugar risk when combined, so both are usually adjusted at the same time.
